Required ForwardHealth Enrollment

All IRIS Vendors must Apply Now with ForwardHealth to continue providing services.

You’re an important part of the care people get through IRIS. You need to apply now as a Medicaid provider through the ForwardHealth Portal to continue working with your clients. Enrollment isn’t complete until your application is approved, which can take up to 60 days. APPLY NOW to avoid payment lapses or delays in 2026.

You enrolled with Wisconsin Medicaid by working with iLIFE. Now, you have to go to the ForwardHealth Portal to apply. This will give you a Medicaid-issued provider ID.

  2. Check out the Enrollment Criteria then click “enrollment application” or go online at www.forwardhealth.wi.gov and click “Become a Provider” in the top left of the screen
  3. Get Connected – You can get notifications sent right to your email. Sign up for the Adult LTC Waiver Provider email list at Subscriptions
Why do I need to enroll in ForwardHealth?

The enrollment process changed because federal law changed.

A federal law called the 21st Century Cures Act required states to have more control over Medicaid provider enrollment. And, the Affordable Care Act required states to look at providers and vendors more closely. The new enrollment process connects providers and vendors directly with DHS for background checks, credentialling review, and enrollment.

If you have more questions or need help you can call Provider Services at 800-947-9627. They are available Monday – Friday 7am-6pm. When you call, say “LTC Waiver” at the menu prompt to talk with someone about enrollment.

• Start by reading the Enrollment Provider Checklist. Then, go to the Enrollment Criteria on the ForwardHealth website and click on “enrollment application” to fill out the required forms.

• After you receive your Medicaid-issued provider ID from ForwardHealth, visit the iLIFE Forms page, choose Vendor, and complete the iLIFE forms.
