Required ForwardHealth Enrollment
You must submit your enrollment or revalidation application by December 31, 2025, to be paid for dates of services on and after April 1, 2026
Wisconsin Medicaid has changed provider enrollment requirements. All our providers delivering Medicaid waiver authorized services must enroll or revalidate with Wisconsin Medicaid through the ForwardHealth Portal by December 31, 2025. It can take several weeks for ForwardHealth to review and approve your application. You must start this process now so your application is approved and your contracts and services are authorized by March 31, 2026. This will allow you to get paid next year.
If your enrollment or revalidation is not approved by March 31, 2026, you will:
- Not continue to receive payment from us.
- Not get reimbursed by Wisconsin Medicaid for dates of service on and after April 1, 2026.
- Lose your contract with the MCO or IRIS participants you work with.
- Lose your IRIS authorizations, if applicable.
- Lose business from people covered by Medicaid.
- Disrupt services for the people who depend on you, who will have to find other providers.
- Worsen the provider shortage in Wisconsin, and cause more strain on remaining providers.
You cannot backdate your enrollment to a date before you submitted your application. And, you cannot hold Medicaid members and families liable for paying for Medicaid services you don’t get reimbursed for.
What you need to do
Most providers should submit a new provider enrollment application on the ForwardHealth Portal to get a Medicaid-issued provider ID. Use the Enrollment Checklist
- Go to the Provider Enrollment for Adult Long-Term Care page.
- Select Start Provider Enrollment Application under Quick Links.
- Follow the prompts to apply.
Supportive home care agencies with EVV-only provider Medicaid IDs have a quicker process. They’ll upgrade their EVV-only enrollment to full Medicaid enrollment. It’s called revalidation. Use the EVV Revalidation Checklist

Why do I need to enroll in ForwardHealth?
The enrollment process changed because federal law changed.
A federal law called the 21st Century Cures Act required states to have more control over Medicaid provider enrollment. And, the Affordable Care Act required states to look at providers and vendors more closely. The new enrollment process connects providers and vendors directly with DHS for background checks, credentialling review, and enrollment.
